We also offer support co-ordination to help you navigate the NDIS and get maximum value out of your NDIS plan. Our trusted Care Advisors can find, assess and co-ordinate all your services when multiple providers are involved, including developing service agreements and reviews.
Whether you live alone, with family or in shared living, our carers can assist you with your in-home core supports. Our personal care support for daily routines and tasks is discreet and respectful in helping you get ready for the day.
We also help you thrive in your home environment and maintain independence by offering assistance with daily household tasks, gardening and meal prep. Whether it’s 1 hour a month or 24 / 7 ongoing care, our core supports help you to live well in your home environment.
Connection to community is so important and our carers can provide reassuring support to ensure you remain active and engaged. We can provide core supports such as travel and transport to appointments, help you participate in community, sporting and cultural events, group/centre activities and shopping, provide companionship, help you visit family and friends and support your volunteer and work participation. However you like to spend your time, we’re here to support you every step of the way.
If your individual support needs are complex, our registered and enrolled nurses can provide expert community nursing care. From complex bowel care, enteral feeding, tracheostomy management and catheter management through to medication management, wound care and ongoing health monitoring, our nurses provide the expert care you need in your own home environment.
We are also proud to assist participants with limited communication and behaviour support needs.
It can be challenging to transition through life and we can help you plan for change and develop strategies for success. All our services are tailored to the individual and include support co-ordination, assistance with budgeting and daily planning, assistance with accommodation and tenancy obligations, peer support and individual skill development.
