Following Your Advanced Care Directive to a T

Those who are in the process of dying should consider creating an advanced care directive which documents your wishes for the remainder of your life should you be unable to articulate them yourself. This can include a Values Directive, which includes all the things you wish to be considered in your medical treatment, and an Instructional Directive, which outlines your consent and refusal of specific medical treatments.


Your Values Directive can include things like what matters most to you, who you wish to have a say in your care (partners, family members, friends, etc.), any preferences you have, and any outcomes you would like to avoid.
